Connecting with Omni Pro II from Offsite

After installing a replacement ATT Modem/router I find that I can no longer get connected to my Omni Pro II while away from the house using my trusty Haiku. 
 
I set up the Modem Fire wall Nat/Gaming as follows:

What IP address do I put into the Haiku to access the OP II from offsite?  While on site I use the OP II IP address with no problem but offsite setup/access baffles me.
 
Do I have to create a Passthrough?

You would use your WAN IP address to connect from outside the house after port forwarding. If you have a Dynamic IP, I suggest using a DDNS! If you need any further help, PM me!
