Access keypad output fault

I am using an HAI Access Keypad with an OmniPro II. Wired as per manual. The red LED is on. When I try and turn the output On to change the LED, there is an output fault on the panel. Any ideas on trouble shotting?

cannot find a solution -> is the unit defective if the Red LED is on when the green wire (controlled via an output) is not connected to anything?
