Central Monitoring Basics with POTS line

Could someone explain the basics for Elk wiring and setup for monitoring via a phone line?  I have my Elk panel installed and operational.  I also have the Elk RJSET Telco Jack and In-line surge protector.  I'll most likely use one of the national CS companies like NextAlarm or AlarmRelay.  This will be my first attempt at central monitoring via a POTS line. 

I appreciate the help!
 
The outside phone line comes in to the Elk RJ-31x first then back out to the house wiring. There is a relay in the elk that disconnects the whole house first then connects the elk to the outside line regardless of what inside devices are doing (in so many words)

FYI. Phone lines can be cut. It's always good to have a backup of some sort, like a cellular unit.
 
Depending on if the CS nickel and dimes you or not....
 
Fire alarm requires a minimum of monthly dialer test. You should enable as many reporting options as possible on the M1 (low battery, etc.) and I recommend restoral reports also.
 
Strongly prefer CID format.
