Omnistat not responding to program

I have five Omnistats in my house and 4/5 work flawlessly.  On one stat, i recently started having an issue where the stat does not respond to program changes.  As an example, I have it set back at night ( which works) and then in the morning when the program requires a 3-4 degree rise (which shows correctly as the set point) the stat does not click on to bring the heat on.  It requires of me that I manually bump the temp a few degrees to get the system on.  I can immediately set it back to the set point and it will maintain.  I thought it was a bad stat so I swapped it for a new one, matched the settings from the old to the new, and I have the same issue!!  The stat setting are identical to what i have going on at the other stats that work!!
 
 
I'm at a loss and would appreciate if anyone has any insight or has ran across this issue before.
 
I guess I should have asked as well, are the Omnistats just running in program mode themselves/standalone, or are they all connected to an Omni panel which is controlling them?  If you have 5, are they feeding to a zone controller or does the faulty Omnistat control its own HVAC unit?  Then finally, if it is connected to an Omni panel, there two modes that the Omnistat can run in, full control mode, or temps based on the alarm state.  I'm guessing you are using the latter, but its not completely clear. Also, if connected to a Omni panel, can the Omni "see" it in PC access or from a console when you select "temperature?"  Sorry, lots of questions, but the Omnistats (at least the newer ones) have so many modes of operation, its important to clairify.  
 
So is the set tempurature coming from the Omnistat program, an Omni panel running a program, or the alarm state?  Is it controlling an HVAC unit, or a zone controller which controls a HVAC unit?
 
Apologies for the delayed response.  In the interim though I've started to have this issue on my other stats so..................
 
Stats are running local programs not from the Omni Panel
They are feeding to a Zone controller
Yes the Omni can see it and I can control from PC Access/Superlink

Couple thoughts:
What is your Installation settings | Program Settings | Program Mode, is it None?
How about your EEC Control in the same menu? (I have 5 & 5 here )
How about System Options?  Are you running Conventional / Auto Changeover or something else?
 
As it turns out the Taco Switching Relay board went bad and shorted out.  I've replaced it and, so far, no tsat issue since then.  The Issues came and went without any apparent reason so I'm keeping my eye out to see if they are really gone or will reappear.  Not exactly sure how the Taco switch could have caused my issues but I'm beyond trying to figure this out.

Not sure how a 'Taco Switching Relay' was connected to a OmniStat?  That's not a standard part for HAI/Leviton thermostat installs, I don't think...

Zone board. Stat connects to it and then that connects to the furnace/circulator. Nothing abnormal.
