T568A or T568B

I think all three methods you mentioned are valid options, BSR... trace the wires if you can (which isn't always easy on a patch panel), check out an outlet that's already been connected, or, failing everything else... guess and if that doesn't work then try the other option:) I've used all three methods before.
 
Not that it is a good idea to be sloppy with your wiring, but if you end up using it for network connections, most modern routers will fix a wiring snafu where you have created a crossover wire (by wiring one end A and the other B or vice versa).

Again, it is important to do it right the first time. But I do have one run where I messed it up and I haven't even bothered fixing it yet because it is only used for a network run and the router corrects my error.
