@ Pete - I did checked all the connections and all were tight so I don't think that was the cause of my problems.
@ Ano/Robbin - my breaker box is in an air conditioned location so I doubt the internal temperature varies more than 65F-85F so I don't think this was the cause of my problems.
As additional info the Phase B led eventually went out again. It seems that the led bulb must be loosely connected to the circuit board, when I squeeze the 3 phase box the phase B led comes on again so I think its just a loose connection to the led bulb It seems to be working alright since I re-added to the network. While reviewing the documentation I see the PSC phase repeater comes with a 5 year warranty so if it acts up again I'll seek a replacement under warranty.
