That's the exact wiper motor I use for my Axworthy ghosts/ghouls. I mounted a 5" plastic pulley on the output shaft itself, but for faster movement, a larger pulley would be better.
The wiper motor has two contacts in a shielded connector. These two connections are used one at a time - one for high speed, one for low speed. The chassis itself is the negative side of the DC power (negative ground in a car). The connector is the positive side - use only one.
I used a 12VDC power supply I built a while back, but anything with 1.5 amps or better should would work.
